I acknowledge that I have voluntarily chosen to receive spa, wellness, and/or aesthetic treatments provided by The Island Spa.

IMPORTANT NOTICE

Spa, wellness, and/or aesthetic treatment can be physically challenging and carry with it risks that we cannot entirely eliminate. These include the risk of personal injury from slips, falls, allergic reactions to treatments, skin conditions and in some circumstances minor abrasions or bruising of the skin following treatment.

RULES

• You must act responsibly and sensibly at all times.

• You must not participate if you are under the influence of alcohol or non-prescription drugs.

• You must inform us of any pre-existing medical condition, allergic reaction, skin or health concern which might reasonably affect your fitness to participate in spa, wellness, and/or aesthetic treatments .

• You must follow any safety warnings or instructions displayed or given to you by a member of staff. Ask a member of staff if you are unclear.

• Wet surfaces can be dangerous, you must not run inside the facility and must take care when moving around the facility, particularly if the floor surfaces are wet.

• We are not qualified to express an opinion that you are fit to safely participate. You must obtain professional or specialist advice from your doctor before participating.

• A list of ingredients contained in cosmetic and other products used and administered at the facility is available on request. It is your responsibility to request and confirm whether any product we use or supply in the course of spa, wellness, and/or aesthetic treatments is safe to be administered to yourself.

• You must not bring your own food or drink

In the absence of any negligence or other breach of duty by us, participation in spa, wellness, and/or aesthetic treatments activity is entirely at your risk.

2.DISCLAIMER FOR PERSONAL INJURY OR DEATH (USE OF GYM EQUIPMENT)
You are responsible for using all equipment and products safely and as directed. You must behave sensibly and follow any safety instructions so as not to hurt or injure yourself or others.

In the absence of any negligence or other breach of duty by us, the use of our equipment, products or facility is entirely at your own risk.

3.DISCLAIMER FOR LOSS OR DAMAGE TO PROPERTY
In the absence of any negligence or other breach of duty by us, we are not responsible for any theft, damage, destruction or loss of your property or belongings while using our facilities.

IT IS IMPORTANT THAT YOU READ AND UNDERSTAND THIS STATEMENT, IF THERE IS ANY TERM THAT YOU DO NOT UNDERSTAND, THEN PLEASE DISCUSS IT WITH US BEFORE AGREEING TO YOUR TREATMENT
